Jeremy Perkins, 32, of Middlesboro, formerly of Coxton, passed away Wednesday, September 26, 2012, from injuries received in a mining accident in Tennessee. Born December 15, 1979 in Harlan County, he had been a coal miner for 14 years. He attended the Coxton Faith Tabernacle.

Jeremy was preceded in death by his grandfathers, Chester Perkins and Bill Adkins; three uncles, Bruce Perkins, Danny Joe Holbrook and Nick McQueen; an aunt, Linda Adkins; a friend, Kelles Greer, Sr.; and his former father-in-law, Daris Napier.

Survivors include his father and step-mother, Clarence and Kathy Perkins, Coxton; his mother, Mary Adkins, Cincinnati; two children, Jeremy Scott Perkins, Kildav; Alexia Perkins, Somerset; three step-children, Zachery Maggard, Sergio Torres and Ariana Torres, all of Somerset; and three sisters, Sylvia Durham, Charity Sumner and Alicia Perkins, all of Cincinnati.

Also surviving are his grandmothers, Betty Perkins and Frances Adkins, both of Coxton; his great-grandmother, Mabel Mitchell, Evarts; his former wife and the mother of his son, Sandy Sage, Kildav; his former wife and the mother of his daughter, Angel Perkins, Somerset; a host of aunts, uncles, cousins, and other family and friends, including a special friend, James Napier, Verda.

Visitation will be held Saturday from 5 until 8 pm at the Evarts Funeral Home.

Funeral services will be conducted Sunday, September 30, 2012, at 2 pm in the Chapel of the Evarts Funeral Home with the Reverend Stan Nicely officiating.

Burial will follow in the Evarts Memorial Gardens with family and friends serving as pallbearers.
